This is what I use either daily, or want easiest access to if I do need it. And, it may look like Tumblr and Twitter are in the #1 and #2 spots, but I’m actually organized from bottom to top (as I assume most do, right?) for the most part.

Tweetie 2 is my favorite Twitter client at the moment.
Birdhouse is great for drafting tweets while on the subway.
 Foursquare lets me see where my friends are where they like going.
Yelp lets me wander the city and surprise myself with good places to eat.
Words With Friends is a Scrabble-like game that I love to play against my friends.
Of the standard icons, I use Maps and Mail the most, and hardly ever use the Phone and SMS apps.

For a long time I didn’t change anything about my iPhone home screen. So I still keep all the original apps on the first page. I switched some things around, and added Ping and Drop7 (also known as the BEST GAME EVER!)
Plus I put text messaging on the bottom becuase I use it way more then I use my iPod.

It’s a combination of my most used first-party apps, my most used web/social media apps, and a few games. I’ll always have the games I’m currently playing through on the home screen, and Peggle. Have to have Peggle. Always.
For the forthcoming secondscreen.org: I actually don’t organize that at all. I just flick to the right and use Spotlight to launch non-home screen apps.
Oh and my home button shortcut is set to launch the camera.
